Job Overview: We are seeking an experienced and detail-oriented Nurse to join our team as a Part – Time Weekend care manager. In this remote role , you will review hospital clinical documentation to determine if patient meets criteria for admission to a skilled nursing facility. Your assessment will ensure timely, accurate, and compliant admission decisions that support high -quality patient care and optimal utilization of services.

Qualifications:

• Current, Active RN or LPN license in good standing
• Minimum of 2 years of clinical nursing experience
• Strong knowledge of Medicare and Insurance preferred
• Excellent verbal and written communication
• Proficient in reviewing electronic medical records( EMR)
• Self – motivated with strong attention to detail and the ability to work independently in a remote environment.
• Compassionate, patient-focused approach to healthcare.

Work Environment:

• Remote
• Requires high-speed internet and secure workspace.

Compensation:

• Hourly rate based on experience

Key Responsibilities:

• Review hospital medical records, clinical notes, and supporting documentation for patients referred to skill nursing.
• Apply Medicare and insurance – specific criteria to determine eligibility for SNF( Skilled ) admission.
• Communicate findings and clearly to hospital discharge planners, admissions teams, and other healthcare partners.
• Document review outcomes accurately in the required systems and formats.
• Collaborate with admissions and clinical teams to clarify criteria. Address discrepancies ad support seamless patient transitions.
• Maintain compliance with HIPPA and organizational privacy policy.
• Serve as a liaison between patients, family members, and healthcare providers.
